Homeless Demographic Survey for HELP Program

You are invited to take part in a demographic survey about Homeless Empowerment Learning Program (HELP) in Long Beach area. HELP is a health education intervention program developed by AUHS Foundation. The program intends to improve the health conditions of the homeless in Long Beach by providing them on-going education and training on both physical and mental preventive health care.

Consent Form

Your participation will require approximately 10 minutes. There are no known risks or discomforts associated with this survey. The survey results will benefit the homeless in long beach area in terms of education and health care.

Participation is voluntary, and responses will be kept anonymous to the degree permitted by the technology being used.  If you choose to be in the study you can withdraw at any time without adversely affecting your relationship with anyone in the community. Any report of this research that is made available to the public will not include your name or any other individual information by which you could be identified.
